PLEASE NOTE: For a minor child’s participation, a parent or guardian must provide their name and email address on this contact sheet, along with their child’s (children’s) first name.
In addition, a parent or guardian must sign a permission slip for their child (which will be emailed) and return it to: KalightoscopeArt@gmail.com before art camp day.

Our programs are part of the educational focus at Humanity Shines Organization, a 501c3 tax-deductible nonprofit company. All net sales and grants associated with this program will be used to create a dynamic experience for children and adults, develop ongoing aspects of the plan, and implement its local, national and worldwide outreach.
